The Million Hearts® Hypertension Control Announces Champions

The Million Hearts® Hypertension Control Champions are clinicians, practices, and/or health systems that have achieved exemplar rates of hypertension control.

Champions are recognized for demonstrating their commitment to help their patients with hypertension manage their blood pressure – achieving a hypertension control rate of at least 80%.

Million Hearts® is recognizing 24 solo practitioners, large health systems, community health centers, and specialty groups as 2023 Hypertension Control Champions. These Champions were able to achieve blood pressure control for at least 80% of their adult patients with hypertension.
