We have onsite pharmacies at seven of our health center locations, making it easy to pick up your prescriptions when you receive your medical and dental care. Our licensed pharmacists can answer your questions and discuss payment options.

Clinical Pharmacy – Award-Winning Advanced Practice Pharmacists
Patients with complex medical conditions (such as diabetes, high cholesterol and high blood pressure) can work one-on-one with our Advanced Practice Pharmacists, also know as Clinical Pharmacists. Advanced Practice Pharmacists have completed one to two years of post-doctoral residencies to work one-on-one with patients who have complex medical conditions. Additional training includes board certification in ambulatory care and/or certification as diabetes educators.
Through collaborative drug therapy management agreements, Advanced Practice Pharmacists have the ability to change patients’ medications as needed. They see patients with diabetes, heart disease, chronic lung disease, and other medical conditions. As part of the healthcare team, they review medications for safety, effectiveness, cost savings, and drug interactions with other medications, food, and various medical conditions. They also support providers in completing Medicare Annual Wellness Visits which focus on preventative health.
El Rio’s Advanced Practice Pharmacists have received numerous awards for their unique approach to effective management of diabetes, their role in Medicare Annual Wellness Visits, and their participation in local and national initiatives related to pain management and innovative practice models.
Our Advanced Practice Pharmacy Diabetes Program offers:
Education and training for diabetes patients is an important part of care. This program focuses primarily on patients with a hemoglobin A1C greater than 9%, offering one on one visits with the Advanced Practice Pharmacists. We also offer Diabetes Empowerment classes through Health Builders as an additional option for all patients living with diabetes.
